And now we can connect you with your personal health information through LMC FollowMyHealth, an easy-to-use online tool that allows you to manage your health care when it’s convenient for you.
LMC FollowMyHealth gives our patients access to their health information 24 hours a day, seven days a week from any Internet-capable device, helping them to participate in their own care. It’s free, simple to sign-up and completely secure.

To access your personal health information online, you can Enroll by Mail
or request an LMC FollowMyHealth account during a physician office or hospital visit. A valid email address is required.
1. Provide your email address to the registration staff at the hospital or at your physician practice.
2. Follow the enrollment instructions in your LMC FollowMyHealth invitation email.
3. Create your secure online account with a username and password.
