Spend less time in the waiting room when you fill out your forms before your appointment.
You should update this every year, or when your information changes.
Come early to update any necessary information. If you arrive more than 10 minutes after your appointment time, we may ask you to reschedule.
Please do your best to cancel at least 24 hours before your appointment. If you cancel three times without advance notice, we may dismiss you from our practice.
Payment is due the day of service, including copayments, coinsurance and any other charges. If your care isn't covered by insurance, you are entitled to a 25% discount.
For less time in the waiting room, fill out your new patient forms before your appointment.
Call and leave a message with your name, birth date, prescription and strength, pharmacy name and number and your doctor's name. Allow 24 to 48 hours for your request. We won't contact you about the request unless there is a problem.
You may be required to see your provider before we can refill your medication. We also do not accept refill requests from local or mail order pharmacies — you must make the request for these refills.
You'll receive your test results in 7 to 10 days by mail or telephone.
Allow up to 7 days for your referral to go through. Once it does, our office or the specialist's office will call you with appointment information. If you make your own appointment, please let us know as soon as possible so we can obtain any prior authorizations that may be required.
